Passport Funding


Passport is a program that helps adults 18 years or older with a developmental disability to participate in their communities. It also helps caregivers of an adult with a developmental disability take a break from their caregiving responsibilities.

Passport provides funding for services and supports so adults with a developmental disability can:

  • Take part in community classes or recreational programs
  • Develop work, volunteer, and daily life skills
  • Get temporary respite for their  caregivers


  • People with a developmental disability 
  • 18-years-old or older 
  • Need support to participate in the community while they are still in school, or;
  • Have left school and are living on their own, with family or independently in a supportive living arrangement

Here at Nourishing Hearts we tailor our activities on individual strengths and provide additional support and modifications to meet unique needs of individuals and families. We believe our community is strengthened by diversity and inclusion for all people. 

Nourishing Hearts Wellness Care Farm offers a quiet and safe environment for people with all different abilities. The farmstead allows for many opportunities for outdoor activities, exercise and contact with animals and nature. The care farm nourishes a sense of community, provides a place of belonging, acceptance and opportunity for meaningful engagement.

* An assessment will be completed to determine if a personal support person is required for participation. Participants are responsible to bring their own support person.
